Description :

Summary : House Manager is responsible for performing essential care for our members in our group homes. This is a safety sensitive position.

Essential functions : To oversee the daily operation and staff of three traditional group homes located in the East Valley. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Reports to the Traditional / DTA Operations Manager and to the Assistant Traditional Operations Manager

1.Responsible for the Residents of the home which include :

Attending and preparing for ISP meetings and attending ISP’s per the request of the Group Home Manager or

Assistant Director.

Maintains ongoing communication with parents and guardians and DDD Support Coordinators

Conduct weekly Resident File audits to maintain accurate, complete and comprehensive resident files

Ensure Resident files are complete and updated with all current information

Ensure all applicable regulation / standards are adhered to at all times

Report to Program Coordinator and COO all incident reports immediately

Ensure residents personal belongings, wheelchairs, and equipment is clean and ready for use

Transportation as required for appointments, outings, etc.

2.Responsible for Medical Aspects of Care which include :

Responsible to the Program Coordinator and Director of Residential Services for reporting all changes of conditions

regarding the residents in the home

Transporting residents to Doctor Appointments

Ensuring all follow up and communication with the Program Coordinator and / or Director of Residential Services

regarding Doctor appointments is completed

Completing Medication Administration Records (MARs) sheets before the 1st of every month.

Verification the MAR match the Physician’s Orders and the labels of all medications

Completing PCSP goals for members are ready before the 1st of the month.

Ensure all required consents are signed by the appropriate guardian / parent?Must perform duties following safety standards required for this position including but not limited to use of PPE’s

Personal Protective Equipment i.e. gloves)

3.Responsible for Employees which includes :

Completing staffing schedules for group home personnel

Ensuring proper client to staff ratio

Additional shift coverage as the need arises for call outs, etc.

Ensuring all group home staff accurately complete the required shift documentation on a daily basis

Train with Program Coordinator all new / existing DSP’s

Assisting to ensure all employees maintain compliance in regard to

4. Responsible for Physical Plant (Group Home) which includes :

Being flexible in hours worked in order to assist with shift coverage if needed

Weekly walk through of the home

Assisting with making sure that the home is ready for surveys and monitoring

5.Responsible for maintaining Group Home Budgets which include :

Grocery shopping

Client Funds

Petty Cash

6.Responsible for maintaining Group Home Van which includes :

Ensuring Van is current on routine maintenance

Ensuring that the group home vehicle has current registration

Ensuring that the Daily / Weekly checks are completed

Must have 1 years’ management experience (Residential group home experience preferred),

Flexible hours, reliable transportation to work at any location.

Must have or be capable of passing and maintaining Article 9 and Prevention and Support within 90 days of hire.

Responsible for Members which includes :

  • Provide for the safety, emotional and physical needs of all members
  • Ensure members are clean, well-groomed and dressed age appropriately and correctly for the season and environmental conditions
  • Transportation to and from day program as needed
  • Position and re-position every 1 hour while awake and every 2 hours while asleep and as needed
  • Help members cook meals and to eat as needed
  • Ensure daily that members are being emotionally and physically stimulated to the best of their ability
  • Assist with showering members to ensure a complete shower is given as needed
  • Responsibility for Medical Aspects of Care which includes :
  • Responsibility to the Nursing Supported Group Home Manager and Chief Nursing Officer for reporting all medical aspects of care and changes in conditions regarding the members in the home
  • Performing routine nursing duties, including :
  • Taking a recording temperature, pulse, and blood pressure
  • Comfort, grooming, and hygiene needs
  • Diet management
  • Collecting specimens
  • Patient activities
  • Passing medications
  • Reordering medications as needed (or ensuring Nursing Services Director is aware and needs to reorder)
  • Assisting in direct care of members to include, but not be limited to :

a. Showering members b. Changing / Repositioning members c. Cooking meals d. Assisting in cleaning the home e .Assisting in doing laundry

  • Safeguard equipment and supplies, and checks equipment apparatus or appliances furnished to resident in the course of treatment
  • Maintaining and ensuring that all documentation is completed before end of shift which includes but not limited to : incident reports, daily nursing notes, Med Sheets, etc.

  • Minimum Qualifications :

    Maintain standards of care as outlined in the Nurse Practice Act by Arizona State Board of Nursing

  • Flexible hours to work times as well as work locations to assist with staff coverage if needed
  • Reliable transportation to work at any location
  • Must have or be capable of passing and maintaining Article 9 and Prevention and Support within 90 days of hire
  • Must have or be capable of passing and maintaining CPR within 30 days of hire
  • Must have or be capable of obtaining and maintaining a Class 1 Fingerprint Clearance Card
  • Job Requirements : For HR use only

  • Must be at least 21 years of age.
  • Must be able to maintain a class 1 fingerprint clearance card.
  • Must abide by company policies and procedures
  • Employees must be able to verbally communicate in English, i.e. be able to understand what is being said to him / her in English. Must be able to use the English language so that what he / she is saying to another person can be understood, in all emergency situations so that the safety of patients / clients, other employees and other individuals who are in our facilities conducting is not jeopardized. All employees must be able to read and comprehend English.
