Position Title: Life Coach
Reports to: Life Coach Coordinator
Position Status: Fulltime, Non-Exempt
Schedule: Tuesday-Friday, 9-5pm with flexibility to work some nights and weekends.

What You’ll Do
As a Life Coach, you will be the primary point of contact for individuals enrolled in the Legendary Legacies. You’ll provide outreach, engagement, assessment, and individualized support to help participants achieve personal and professional stability.
Your Key Responsibilities:
Participant Support & Coaching
- Build relationships with participants through consistent outreach, mentorship, and coaching.
- Use a person-centered approach to develop Individual Service Plans (Legacy Plans) tailored to participants’ goals.
- Provide support, guidance, and accountability to help individuals navigate challenges related to housing, employment, recovery, and reentry.
- Help participants set and achieve realistic, meaningful goals for their personal growth.
Care Coordination & Linkages to Services
- Connect participants to mental health, behavioral health, and substance use services as needed.
- Work with outside agencies—including hospitals, police, parole officers, and social service providers—to ensure wraparound support.
- Maintain clear, accurate documentation on participant progress, service plans, and engagement efforts.
Community Reintegration & Advocacy
- Help participants navigate the reentry process, including securing housing, employment, and identification.
- Serve as an advocate and liaison between participants and external providers.
- Facilitate life skills training, job readiness coaching, and goal-setting workshops.
- Provide transportation assistance to critical appointments when necessary.
Crisis Response & Problem-Solving
- Act as a first point of contact for participants in crisis, connecting them to immediate resources.
- Support safe transitions in care for members moving between different treatment, residential, and community settings.
- Work closely with program leadership to identify and remove barriers to success.
Documentation & Continuous Improvement
- Maintain accurate, up-to-date participant records in compliance with program standards.
- Assess progress against service plan goals and adjust strategies as needed.
- Attend and participate in training, team meetings, and community collaborations.
- Utilize the Life Coach manual as a tool for best practices.

Minimum Requirements:
- High School Diploma/GED required; Bachelor’s degree in a health-related field preferred.
- Experience working with individuals navigating reentry, mental health, and/or substance use recovery.
- Ability to connect participants to services—knowledge of local resources, behavioral health systems, and social services is a big plus.
- Strong relationship-building and mentorship skills.
- Excellent documentation, time management, and organizational skills.
- Valid driver’s license & reliable transportation (travel to community sites is required).
- CPR & First Aid Certification (or ability to obtain).
- Preferred: Spanish-speaking ability to better support our diverse participants.

Pay: $24.00 - $27.00 per hour
