Job Summary
We are seeking a compassionate and dedicated Registered Nurse or LPN to join our healthcare team. This role involves working with patients in memory care, home care, and hospice palliative medicine, ensuring their comfort and well-being while adhering to best practices in nursing.
Responsibilities
- Assess patient health problems and needs, developing and implementing nursing care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patient progress and maintain accurate medical records using electronic health record systems such as Epic and Athenahealth.
- Provide education and support to patients and their families regarding health conditions, treatment options, and self-care strategies.
- Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
- Perform laboratory tests and procedures as required, including the ability to dialyze patients when necessary.
- Deliver care in various settings, including ER, primary care, memory care facilities, and home environments.
- Ensure compliance with safety protocols and infection control measures.
Skills
- Strong clinical skills in areas such as ER, primary care, home care, memory care, hospice palliative medicine. No experience required
- Excellent communication skills for effective patient interaction and collaboration with healthcare teams.
- Ability to perform laboratory tests accurately.
- Knowledge of patient care techniques for inhalation therapies.
- Compassionate approach towards patient care with a focus on empathy and support.
- Strong organizational skills with attention to detail in documentation and patient management.
