MEDICAL SOCIAL WORKER (MSW) HOSPICE – FULL TIME DAYS
Summary
Provides psychosocial assessment, counseling, crisis intervention, and information / referral services on a consultation basis. Works collaboratively with the management team to ensure the efficient utilization of resources for positive patient outcomes.
Job Duties
- Employs counseling skills in order to help patient / families understand, accept and follow recommendations for care and to restore patient to optimum social and health adjustment.
- Stays up to date on current trends and researches the availability of community agencies and community resources for social, emotional, and financial assistance.
- Intervenes in crisis situations and effectively applies crisis intervention theory techniques.
- Identifies and records clinical, psychosocial and financial barriers to a smooth transition across the healthcare network.
- Participates in a monthly or yearly audit or review to ensure adherence to laws and other federal guidelines.
- Works collaboratively with the patients and patient's social support system to establish and implement a plan.
- Interviews the patient to complete the initial psychosocial evaluation.
- Reports suspected cases of child and adult abuse / neglect / exploitation.
