Qualifications:
- Has a minimum of a bachelor’s degree from an accredited university or college degree in social work, counseling, psychology, sociology, rehabilitation services, special education, early childhood education, secondary education, family and consumer sciences, criminal justice, or human growth and development.
- Has at least one year of experience working with individuals with mental/behavioral health disorders.
- Completes and is right standing with all pre-employment and post-employment requirements specific to all governing entities.
Major Task List:
- Provides community support and rehabilitation services to clients as assigned to caseload.
- Participate in the service and treatment planning as an active team member.
- Participate in transfer, transition, and discharge planning efforts specific to assigned clients.
- Provides only prescribed services indicated in person-centered treatment plans specific to staff type and service type.
- Provides individual Psychosocial Rehabilitation Skills Training and supportive intervention utilizing a restorative and rehabilitation approach to service delivery.
- When prescribed, co-facilitate PSR Groups with fully licensed and/or provisionally licensed qualified staff to eligible clients with group prescription of services.
- Participates in staff training assignments, as requested.
- Collaborates with team members when co-jointly assigned a client in promoting and rendering continuity of appropriate and coordinated care based on prescribed service types.
- Assist in gathering information for assessment, review, and reassessment processes for assigned clients.
- Identify, report, and maintain weekly appointments for community resources.
- Complete accurate, comprehensive, and appropriate progress notes based on rendering of services while adhering to the agency’s policies and procedures and other governing entities guidelines.
- Assumes lead role if the case is not co-jointly assigned pertaining to crisis intervention and crisis mitigation specific to the assigned client’s needs. If co-jointly assigned, the clinical supervisor will determine what staff type should assume lead role.
- Participate in the coordination of arranging transportation for client to psychiatrist for psychiatric evaluations and/or follow-up medication management and monitoring appointments.
- Serves in multi-purpose human service role to ensure that client has access to appropriate care with other identified professionals or organizations based on presenting concerns and/or other concerns that the agency is not licensed to provide.
- Consistently engage in professional code of conduct and adhere to all agency’s policies and procedures.
- Participate in clinical supervision as scheduled.
- Participate in assessment of competencies and performance appraisal processes.
- Participate in internal and external professional development opportunities.
- Adheres to the agency’s team approach philosophy through collaboration and consultations with colleagues/team members.
- Works independently, as needed.
- Adhere to the agency’s communication correspondence policies and procedures specific retrieval and response to electronic, written, oral, or telephonic methodsof communication.
- Attends all scheduled staff training, team meetings, etc.
- Understands, acknowledges, and agrees to adhere to regulatory guidelines of the agency, Louisiana Medicaid, LDH, CARF, and other relevant stakeholders.
- Acknowledges responsibility to participate in agency growth.
- Participates in the agency’s Performance Improvement initiative.
- Assists with other projects related to agency operation, as requested.
