What are the responsibilities and job description for the Experience Hospice Chaplain / Spiritual Counselor needed - PRN position at Lenity Light Hospice?
We are looking for an emotionally intelligent Spiritual Care Counselor/Chaplain to act as advocate and provide support for our terminally ill patients. Must be able to think outside of the box (we have a large Palliative Care Program), self sufficient. Must be comfortable with all faiths providing spiritual and non spiritual care counseling as needed for patients.
Good understanding of identifying needs patients and families may have and passing this information onto nursing and IDT team.
This is a part time/prn position with opportunity to go full time as our census grows.
Responsibilities
- Plan, coordinate, manage and implement support packages to help clients deal with difficulties and overcome dependencies
- Provide spiritual and nonspiritual counseling to terminally ill patients and their families.
- Understand and be comfortable with the dying process.
- Provide psychosocial assessments as required by CMS guidelines for patients admitted to hospice care. Be able to perform spiritual assessments as needed per CMS guidelines.
- Address each case as a unit and set tailored measurable goals
- Monitor and evaluate patient’s progress and modify treatment plans accordingly
- Offer information and counseling on the best course of action during sessions
- Act as a key-worker and cooperate with multidisciplinary teams
Skills
- Proven work experience as a spiritual care counselor or chaplain, minimum of 2 years in hospice or palliative care.
- Master Level in divinity or equivalent training.
- Social perceptiveness and empathy
- Ability to build and maintain professional helping relationships
- Ability to relate and communicate with diverse population and groups
- Resilience along with ability to assess situations
- Willing to submit to background checks
- Comfortable with the dying process, and able to teach and offer comfort during this difficult time to patients and their families.
Pay: $20.00 per hour
