Reporting Relationships/Supervisory Responsibilities
The HIM Data Integrity Specialist reports to the HIM, Manager and has no direct reports.
Position Summary
The HIM Data Integrity Specialist is responsible for improving the integrity of data by identifying, investigating, and correcting errors within the electronic medical record system. Manages MRN duplicates and overlays, assisting with electronic chart corrections, resolving assigned HIM interface errors, performing electronic charting investigations and overseeing the chart correction processes in the EHR. Coordinates with the parties involved in each chart correction case and for ensuring all necessary corrections are made and each case is validated prior to closure. Assigned to special projects and other duties as assigned. Serve as mentor to less experienced staff, providing assistance when required and reviewing their work as directed. Must be able to travel to all ProMedica facilities as needed. Position is remote eligible after training and maintaining productivity standards - must be within 100 miles of Toledo.

Responds to chart corrections requests from clinicians, hospital departments, patients, and families in a timely manner. Educates employees on reporting and correcting mistakes in health record documentation. Maintain understanding of HIPAA and other compliance and regulatory requirements related to correct clinical data and patient identification. Facilitate and complete HIPAA patient amendment requests ensuring that the designated procedure is followed accurately and timely while meeting federal guidelines. Prepares meeting materials and conducts meetings to educate staff and collaborates with both intradepartmental teams and external department representatives on issues related to patient integrity. Researches and analyzes potential MPI duplicates and merges medical records according to procedures. Disseminates merge and MRN changes to all downstream systems and departments. Determines patterns of redundant MRN creation and communicate findings to HIM management and other departments, as necessary. Maintains and updates demographic information in the MPI system to ensure the most current and accurate identification of each patient. Monitors and evaluates patient demographic overlay changes to ensure integrity of patient information. Analyzes and reports trends in erroneous demographic changes. Investigates reports of mis-identity or identity theft in close collaboration with the Privacy Office. Identifies and research charting errors, including incorrect patient selections and rectify MPI compromises using chart correction functions such as contact moves, overlays, unmerges, and documentation revision. Coordinates correction efforts with other affected departments by assigning and verifying completion of chart correction tasks. Audits historical medical records to ensure proper procedures were followed. Works closely with departments to identify and resolve complicated documentation issues. Develops and executes reports, provides support and data for systems improvement initiatives, and contributes to information quality and integration projects. Performs all other duties as assigned.

Education: Bachelor’s Degree in healthcare related field or equivalent combination of education, training, and experience.
Skills: Must demonstrate proficiency in HIM database systems, Electronic Health Record (EHR), and Electronic Document Management systems. Must demonstrate proficiency in medical terminology. Must be proficient in Office software (i.e., Adobe, Word, Excel, and Outlook). Ability to analyze, organize and prioritize work under pressure while meeting deadline. Must possess good interpersonal communication and strong customer service skills both verbally and in writing, including knowledge of basic grammar, spelling, and punctuation. Must be able to work with others to fully understand the scope of data integrity capabilities within the EHR. Ability to lead and facilitate meetings effectively, ensuring they stay focused on the agenda, involve all participants, and achieve desired outcomes. ProMedica Internal Only
