- We provide gender affirming care, medication assisted treatment, and work closely with two assigned social workers,
- We have a behaviorist on site to serve as a bridge into behavioral health services.
- We are a patient centered medical home.
- Spanish speaking physician preferred
- Nurse triage takes first call for this practice
- Physician-designed compensation model; not RVU based

Qualifications
Minimum Education:
- Doctor of Medicine (MD) Required or
- Doctor of Osteopathic Medicine (DO) Required
- Licensed Medical Physician and Surgeon Upon Hire Required or
- Licensed Doctor of Osteopathic Medicine Upon Hire Required and
- Basic Life Support Upon Hire Required
- Diplomate of the American Board of Family Medicine or the American Osteopathic Board of Family Medicine, or Board eligible. Upon Hire Required and
- Completion of a Family Medicine residency. Upon Hire Required
- Administrative skills.
