A Community Health Worker (CHW) acts in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW’s work in a team-based structure and spend most of their time in the community engaging directly with members.
Schedule: Must be available to work Monday through Friday, 8 AM to 5 PM - ability to work alternate hours outside of normal business hours as needed.
Travel: up to 75% of work time. Coverage area is Baton Rouge, LA and surrounding areas up to 100 miles.

Primary Responsibilities
- Create a positive experience and relationship with the member
- Help member set goals and develop a plan of care to achieve those goals
- Proactively engage the member to manage their own health and healthcare
- Support the member to maintain their health and wellbeing by attending regular visits to their primary care provider (PCP) as well as assisting member in continuing to improve their health to stay out of the ER and hospital
- Assist member in engaging with mental health and substance use treatment (as needed)
- Provide member education on community resources and benefits
- Assist the member in accessing transportation services to ensure they attended scheduled medical appointments
- Conduct post discharge activities from hospital and support connection to social services
- Support member to engage in work or volunteer activities, if desired, and develop stronger social supports through deeper connections with friends, family, and their community
- Collaborate/Partner with other members of the members care team (community, providers, internal staff)
- Continue advancing knowledge and learning of community cultures and values
- This position is a field-based position with a home-based (telecommuting) office. You will work from home when not in the field

Required Qualifications
- High School Diploma/GED (or higher)
- Active, unrestricted LPN license in the state of Louisiana
- 2 years of Case Management experience with knowledge of the resources available in the community
- 2 years as a Licensed Practical Nurse (LPN)
- 2 years of field-based work experience (going into the home/environments of members being served)
- 2 years of experience with knowledge of the Medicaid and/or Medicare population
- 2 years residing within the local community
- Ability to travel locally, up to 100 miles round trip and up to 75% of work time
- Must be a resident of Louisiana
- Valid state of Louisiana driver’s license and clean driving record
- Must have your own means of transportation with automobile insurance in place
- Access to a designated workspace inside your home with access to high-speed internet availability and comply with UHG Telecommuting Policy
- Bachelor’s Degree (or higher) in a health-related field
- Bilingual
- 2 years of knowledge and/or experience working with medically complex members with addiction and/or behavioral health conditions
- 2 years of experience working in Managed Care
- Knowledge and /or experience with behavioral health or substance use disorders
