Summary
- The Heart Failure (HF) Coordinator works with the interdisciplinary health care team to facilitate patient understanding of the disease process and to promote care based on evidence based clinical practice guidelines. The position requires expert communication skills and the ability to work collaboratively with a multi-disciplinary health team. The HF Coordinator maintains a strong component of patient advocacy and provides both formal and informal education for professional, technical and ancillary personnel.
- Evaluates identified patient records for compliance with evidence-based standards of care by reviewing physician and nursing documentation and diagnostic studies.
- Initiates face to face communication with nurses, attending/consulting physicians and other health care providers to discuss, confirm or make suggestions regarding patient management and evidence-based standards.
- Initiates or recommends referrals (consultations) to physicians and/or other health care providers (social services, case management, pharmacy, palliative care, nutrition, physical therapy, cardiac rehabilitation, etc.) and provides information regarding community health programs to support risk management (smoking cessation classes, etc.)
- Interfaces with family, therapists, case managers and social workers as needed, to identify specific discharge needs, facilitating optimal care transition and preventing re-admissions, Emergency Department visits and unscheduled office visits.
- Takes accountability of communication for post discharge follow up phone calls to patients discharged with a primary diagnosis of HF.
- Identifies opportunities for and provides both formal and informal education for nursing and other health care providers.
- Regular visits to key departments inclusive of medical/surgical (M/S) departments, critical care (CC) areas, the emergency department (ED) and support services for the purposes of providing feedback, discussing concerns, and identifying opportunities for departmental education.
- Participates in community outreach via health screenings and educational presentations as needed.
- Remains current with American Heart Association (AHA) guidelines for the management of patients with HF, and with measures/standards set forth by The Joint Commission.
- Assumes accountability for Get With The Guidelines (GWTG) Heart Failure, Target Heart Failure and other initiatives related to patients with a heart failure diagnosis.
- Participates in system wide HF initiatives inclusive of Registry Meetings, HF Disease Management meetings and others as requested/appropriate.
- Registered Nurse - NJ (Required)
- Must document a minimum of 8 hours of heart failure specific education and an additional 8 hours of other cardiovascular related education annually.
- Cert Heart Failure RN
- Heart Failure Certification through the American Heart Association of Heart Failure Nurses
- Bachelor of Arts or Science (Required)
- Master of Arts or Science
