A Few Must-haves For This Position
- Current license as an Licensed Practical Nurse (LPN) in the State of MN.License is active and in good standing.
- 2 years' experience as an LPN.
- Able to travel within geographic area serviced by the office.Valid driver's license, reliable transportation, and proof of auto insurance.
- CPR Certification

- Flexible scheduling of patient visits.

- Participatein the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s).

- Recognizes and reports changes in the patient’s condition to the RN.
- Counsels and educates the patient and family in meeting healthcare related needs.
- Completes timely and accurate clinical notes including addressing patient’s progress.
