Letcher Manor is seeking a qualified Social Services Director to join our team. We're looking for applicants who are highly motivated, energetic, professional, and possess clinical knowledge as well as excellent interpersonal skills. Experience in the long-term care setting is highly preferred. The ideal applicant will be skilled in establishing and maintaining therapeutic relationships with residents. Social Services Director Responsibilities:
- Complete forms, reports, evaluations, studies etc. related to social services.
- Assist with resident admission into the facility
- Develop a post-discharge plan
- Develop a workable quality assurance program for social services and coordinate this activity.
- Retain a listing of and be knowledgeable of, all available resources needed for comprehensive care needs of residents, to include community agencies.
- Process all abuse and grievance complaints and work with local social services agency in resolution of abuse, keeping administrator aware at all times of progress and process. Initiate filing adult protection forms when indicated.
- Maintain periodic content with family and schedule regular individual conferences with residents.
- Responsible for notifying legally responsible parties of the residents of impending quarterly evaluations of each resident’s care plan and request their input or attendance. If the resident’s condition should warrant more frequent evaluations, the Social Services Director will be responsible for the notification.
- Assist with pre-admission screening.
- Participate in all care plan conferences.
- Counsel resident, family members, and staff.
Social Services Director Requirements:
- Licensed Social Worker preferred.
- Minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field, including, but not limited to, sociology, gerontology, special education, rehabilitation, counseling, and psychology.
- One year of supervised social work experience in a health care setting working directly with individuals.
- Long-term care experience preferred.
