Licensed Social Worker (LSW) – Part-Time (Adult Program)

Location: Toledo, OH
Schedule: Monday – Thursday, 4:00 PM – 6:15 PM (with additional hours for assessments and counseling)
Compensation: $35/hour (No benefits at this time, but may be offered as the organization grows)
Position Overview:
Leveling Up Therapeutics is seeking a Licensed Social Worker (LSW) to support our adult mental health (MH) and substance use disorder (SUD) clients. This is a part-time position focused on conducting diagnostic assessments, facilitating group therapy, and providing individual counseling.
On days when the LSW is conducting diagnostic assessments (DX assessments) or individual counseling, a Qualified Mental Health Specialist (QMHS) will facilitate the group session. All documentation must adhere to CARF standards, as we uphold a strict documentation policy.
Key Responsibilities:1. Diagnostic Assessments (DX Assessments)
- Conduct comprehensive assessments for new adult clients.
- Develop individualized treatment plans based on assessment findings.
- Ensure compliance with CARF accreditation and Medicaid standards.
2. Group Therapy Facilitation
- Lead therapeutic group sessions for adults focused on mental health and substance use recovery.
- Use evidence-based interventions to promote coping skills, relapse prevention, and emotional regulation.
- When conducting DX assessments or individual counseling, the QMHS will facilitate group in the LSW’s absence.
3. Individual Counseling
- Provide one-on-one therapeutic support to clients as needed.
- Utilize trauma-informed care and motivational interviewing techniques to support client progress.
4. Documentation & Compliance
- Complete progress notes, treatment plans, and other required documentation in compliance with CARF and Medicaid standards.
- Maintain accurate and timely records to ensure compliance with agency policies.
Qualifications:
- Active LSW License in the state of [Insert State].
- Experience working with SUD and MH populations preferred.
- Familiarity with CARF standards and Medicaid documentation requirements.
- Strong clinical assessment, counseling, and group facilitation skills.
- Ability to work independently while collaborating with a multidisciplinary team.
