The Rehabilitation Specialist is responsible for the comprehensive services offered through on-site and off-site programming providing evidence-based support in community settings. This includes assessment, case management, behavioral support services, psycho-education, crisis intervention, advocacy, and community liaison. The Rehabilitation Specialist is also responsible for the timely and accurate completion of all documentation such as treatment plans and progress notes. Staff is responsible for assessing and teaching community skills including Activities of Daily Living as needed. Rehabilitation Specialists will submit all necessary documentation in accordance to COMAR regulations and promptly according to agency guidelines. Rehabilitation Specialists are responsible for maintaining their level of skill by completing the required 8 hours of training per year above and beyond the required basic COMAR training. In addition, each employee is expected to abide by all safety rules and regulations of the department and facility. The Rehabilitation Specialist is responsible for carrying out the mission, philosophy of care and policies of LEWC Inc. The Rehabilitation Specialist also performs other duties as needed to achieve program goals and objectives.
Competencies
1. Writing, interpersonal, and oral communication skills
2. Knowledge of relevant symptoms of the most common diagnosis within the PRP programs i.e. schizophrenia, bipolar, borderline personality disorder
3. Proficiency in assessments, treatment planning, advocacy skills, and case management skills
4. Basic knowledge of Microsoft Office Programs
5. Demonstrate an understanding and sensitivity to serving culturally diverse and/or special needs population
6. Patient confidentiality

Work Environment
This job operates in a professional office environment requiring routine use of standard office equipment as well as out in the community.
Physical Demands
In order to perform the duties of this job, the employee is regularly required to communicate and hear. The employee is required to stand, walk, use hands and fingers, and reach with hands and arms. The employee is also required to regularly operate a motor vehicle.
Supervision and Coordination of Services
The Rehabilitation Counselor will report directly to their immediate supervisor (the Rehabilitation Specialist), and/or the Program Director. The Rehabilitation Counselor will coordinate with other agency employees, as well as with multiple outside organizations and service providers in order to create an interdisciplinary team approach. The employee abides by the ethics set forth by the agency and the profession as a whole.
Travel
The position requires employees to travel to and from client homes and to other service providers as needed according to their client’s individualized goals as well as to and from the various locations of the organization as needed for training, meetings, and other events as necessary. Employee utilizes their personal vehicle and agency vehicles. A clean driving record is required.
Required Education and Experience
1.) High school diploma
2.) At least 1 year of experience working with individuals with chronic mental health issues (may be substituted with further education) *and/or substance abuse.
Preferred Education and Experience
1.) Bachelor’s degree in Human Services-related field from an accredited institution and/or equivalent experience.
2.) 2 years experience in a mental health setting.
3. DLA-20 Certification - 60 hrs Onsite Online training modules provided
4. CPR Certification-Onsite training provided
