Responsibilities
- Provides service within the scope of practice as defined by state laws governing occupational therapy.
- Evaluates patients' functional status, occupational therapy needs, and consults with physicians in developing therapy plans.
- Observes, records, and reports patients' responses to treatment and changes in their condition.
- Instructs and informs healthcare teams, patients, and caregivers about plans of care, functional limitations, and progress towards goals.
- Visits patients within their respective agency's service area and other nearby LHC agencies as needed.
- Completes progress notes for each visit, submits them per LHC policy, and completes patient evaluations within state-specific guidelines.
- Participates in staff conferences and committees as requested.
- May supervise home health aides per LHC policy if allowed by state regulation.
- Provides supervision of occupational therapy assistants per LHC policy and state regulation.
- Prepares discharge summaries and/or written reports on patients' conditions every 60 days.
- Conducts patient assessments using diagnostic and prognostic functional ability tests.
- Provides in-service education as requested by agency leadership.
- Adheres to LHC Therapy Standards of Practice in providing services to all patients.
- Maintains up-to-date knowledge and skills through education and adheres to state-issued licensure requirements.
Education and Experience
- Current Occupational Therapy licensure in New Jersey
- Current CPR certification
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
