LHC Group is seeking a skilled and compassionate Home Health Occupational Therapist to provide high-quality patient care in the comfort of their own homes.

Responsibilities:
- Assess and evaluate patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, and adaptive equipment.
- Determine treatment plans and perform interventions aimed at improving and enhancing patient well-being.
- Evaluate patient progress and report outcomes to the nurse supervisor and physician.
- Inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress towards goals.
- Visit patients within the entire service area of their respective agency and other nearby LHC agencies as needed.
- Complete progress notes for each visit and submit them per LHC policy.
- Participate in staff conferences and committees as requested by the agency.
- May supervise home health aides per LHC policy if allowed per state regulation.
- Provide in-service education as requested by agency leadership.
- Adhere to LHC Therapy Standards of Practice in provision of services to all patients.
- Access education to maintain and enhance clinical knowledge and skills that are up to date with the current standards of care for the profession and to maintain state-issued licensure.
- Requires schedule flexibility, including participation in weekend rotations and potential availability on observed holidays.
Education and Experience:
- Current Occupational Therapy licensure in the state of New Jersey.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
