Job Title: Home Health Physical Therapist
LHC Group is seeking a highly skilled and dedicated Home Health Physical Therapist to join our team in White Hall, Arkansas.
Key Responsibilities:
- Evaluate patients' functional status and physical therapy needs, assisting physicians in developing personalized treatment plans.
- Develop and implement individualized interventions aimed at improving and enhancing patients' well-being, including relieving pain and restoring function.
- Communicate with healthcare teams, patients, and families/caregivers regarding treatment plans, progress toward goals, and any changes in patients' conditions.
Requirements:
- Current licensure as a Physical Therapist in the state of practice.
- Current CPR certification.
- Must have a current driver's license, vehicle insurance, and access to a dependable vehicle.
