Job Description
We are seeking a highly skilled Home Health Physical Therapist to join our team at Patient Care in Saddle Brook, NJ. The successful candidate will be responsible for providing high-quality physical therapy services to patients in the Northern Bergen County area.
Key Responsibilities:
- Evaluate patient care needs and develop individualized treatment plans.
- Perform interventions to improve and enhance patient well-being.
- Monitor patient progress and report changes to the supervising nurse and/or physician.
Requirements:
- Current Physical Therapy licensure in the state of New Jersey.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
