Job Description:
We are seeking a skilled and dedicated Home Health Physical Therapist to join our team at Patient Care in West Orange, NJ.
This is an ongoing full-time position that requires the ability to work with patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
Responsibilities:
- Evaluate patient's functional status and physical therapy needs, assist physician in developing therapy plan of care, and observe and report patient's response to treatment and any changes in condition.
- Treat patients to achieve optimal outcomes, instruct and inform healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals.
Requirements:
- Current Physical Therapy licensure in state of New Jersey
- Current CPR certification
- Valid driver's license and vehicle insurance, and access to dependable vehicle
