LHC Group is seeking a qualified professional to fill the position of Home Health Physical Therapist in Franklin, North Carolina.
Responsibilities and Requirements
- Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
- Develop and implement a treatment plan aimed at improving and enhancing the patient's well-being.
- Perform interventions and provide ongoing care to patients with varying physical therapy needs.
- Evaluate patient progress and report any changes in condition to the supervising nurse and/or physician.
Requirements:
- Current Physical Therapy licensure in the state of North Carolina.
- Current CPR certification.
- Valid driver's license and access to a dependable vehicle.
