We are seeking a compassionate and skilled Physical Therapy Assistant to join our team at CMH Home Health Care, part of LHC Group. As a Physical Therapy Assistant, you will have the opportunity to build trusted relationships with patients, provide exercise instruction and patient education, and contribute to their overall well-being.
The ideal candidate will be a licensed physical therapy assistant with experience in home care settings. You will be responsible for following established plans of care, performing treatments, and making recommendations for changes in treatment as needed. Your strong communication skills and ability to work independently will enable you to effectively coordinate care with other members of the healthcare team.

Responsibilities:
- Perform treatments and provide exercise instruction and patient education aimed at improving/enhancing patient well-being
- Follow all state-specific laws governing the provision of physical therapy in home care
- Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training
- Observe, record, and report patient responses to treatment and changes in condition to supervising PT, nurse supervisor, and/or physician
Requirements:
- Current licensed, registered, or certified to assist in the practice of physical therapy in state of practice
- Current CPR certification
- Current driver's license, vehicle insurance, and access to dependable vehicle or public transportation
