Key Responsibilities

- Developing and implementing individualized treatment plans to address patient-specific needs
- Providing education and training to patients, families, and caregivers on physical therapy techniques and strategies
- Collaborating with other healthcare professionals to ensure effective care coordination and transitions
- Observing and recording patient progress, making recommendations for changes in the plan of care as necessary
The successful candidate will also possess strong analytical skills, with the ability to analyze data and make informed decisions to optimize patient outcomes.
Requirements and Qualifications
- Current licensure, registration, or certification to assist in the practice of physical therapy in the state of practice
- Current CPR certification required
- Reliable transportation and a valid driver's license
