What are the responsibilities and job description for the Therapy Specialist Position Open position at LHC Group?
The Physical Therapist (PT) in Home Health is a vital member of our team, responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, the PT works with the physician to develop the therapy plan of care, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.
The Physical Therapist is expected to observe and report the patient's response to treatment and any changes in their condition to the supervising nurse and/or physician. Treatments may include relieving pain, developing and restoring function, or maintaining maximum functioning using acceptable standards of practice.
The Physical Therapist also instructs and informs participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
