Position : Rehabilitation Specialist
Location : Tempe, AZ
Compensation : $21 / hr - $26 / hr, depending on experience (DOE)
Sign-On Bonus : $4,000
Field Work : 80% field work, mileage reimbursement will be provided
Job Summary : We are seeking a dedicated and experienced Rehab Specialist to join our client's team. The ideal candidate will have a strong background in rehabilitation services and will provide comprehensive assistance to individuals with serious mental illness (SMI) and behavioral health challenges, helping them achieve their rehabilitation goals.
Key Responsibilities :
Provide individualized rehabilitation support to clients, including developing and implementing rehabilitation plans.
Conduct assessments to identify clients' rehabilitation needs and goals.
Develop personalized rehabilitation strategies to help clients achieve their goals.
Facilitate rehabilitation workshops and group sessions to promote skill development and recovery.
Collaborate with a multidisciplinary team to ensure coordinated and effective care.
Respond to crisis situations and provide immediate intervention as needed.
Maintain accurate and up-to-date client records using Nextgen software.
Participate in rotational on-call duties to provide 24 / 7 support to clients.
Qualifications :
Associate's degree or higher in a related field is strongly preferred.
Minimum of 3 years of experience working with the SMI / behavioral health population.
Experience in rehabilitation services and support.
Strong interpersonal and communication skills.
Ability to work flexible hours, including holidays and weekends.
Experience with Nextgen software and CPRP certification is a huge plus.
Preferred Skills :
Empathy and the ability to build trusting relationships with clients.
Strong problem-solving and crisis intervention skills.
Ability to work collaboratively within a team environment.
Knowledge of community resources and support services related to rehabilitation.
Pay Details : $21.00 to $26.00 per hour
