We are seeking a committed Direct Care Monitor to support individuals experiencing mental health challenges, substance abuse issues, and homelessness. This position provides essential assistance and resources.
Responsibilities :
Deliver support services, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and connection to community providers.
Ensure the safety of residents and the facility.
Qualifications :
Demonstrated commitment to assisting high-acuity individuals.
Minimum of 1 year of case management experience or related experience.
Ability to work collaboratively with clients.
Strong documentation skills.
Experience with HMIS is a plus.
Openings :
Full-time and weekend part-time positions available.
Morning (7 : 00 AM - 3 : 30 PM) and swing (3 : 00 PM - 11 : 30 PM) shifts offered.
Pay Details : $21.00 to $24.00 per hour
