Location : 100% Remote (Supporting Pacific Time Zone) Preferred Candidates : Nevada, Oregon (No California or Washington candidates)
LHH Recruitment Solutions is hiring a Patient Intake Case Coordinator for Transition of Care role with an established healthcare company. This 3 month contract role involves coordinating discharge plans for ambulatory patients, working with care providers, and ensuring smooth transitions. Experience with Epic EHR is required, and candidates must be open to rotating weekends.
Key Responsibilities :
Develop and implement transition of care (TOC) plans with Case Managers and Social Workers.
Communicate with patients, families, and external vendors to ensure timely discharge.
Monitor and document progress towards TOC goals.
Coordinate post-discharge referrals and authorizations.
Facilitate patient transfers to post-acute facilities.
Qualifications :
High School Diploma or equivalent.
At least 1-year recent relevant experience.
Knowledge and experience with Epic EHR.
Strong communication and time management skills.
Ability to work independently and handle a high volume of tasks.
Additional Information :
Must be open to rotating weekends.
Involves handling faxes, voicemails, and data entry.
Pay Details : $18.00 to $20.00 per hour
