Position : Peer Support Specialist
Location : N. Phoenix, AZ
Compensation : $21 / hr - $26 / hr, depending on experience (DOE)
Sign-On Bonus : $4,000
Field Work : 80% field work, mileage reimbursement will be provided
Job Summary : We are seeking a dedicated and experienced Peer Support Specialist to join our client's team. The ideal candidate will have a strong background in peer support and will provide comprehensive assistance to individuals with serious mental illness (SMI) and behavioral health challenges, helping them achieve their personal recovery goals.
Key Responsibilities :
- Provide individualized peer support to clients, sharing personal experiences to foster hope and recovery.
- Conduct assessments to identify clients' needs and goals.
- Develop personalized support plans and strategies to help clients achieve their recovery goals.
- Facilitate peer support groups and workshops to promote community integration and mutual support.
- Collaborate with a multidisciplinary team to ensure coordinated and effective care.
- Respond to crisis situations and provide immediate intervention as needed.
- Maintain accurate and up-to-date client records.
- Participate in rotational on-call duties to provide 24 / 7 support to clients.
