Position : Rehab Specialist
Location : Tempe, AZ
Compensation : $21 / hr - $26 / hr, depending on experience (DOE)
Sign-On Bonus : $4,000
Field Work : 80% field work, mileage reimbursement will be provided
Job Summary : We are seeking a dedicated and experienced Rehab Specialist to join our team. The ideal candidate will have a strong background in rehabilitation services and will provide comprehensive assistance to individuals with serious mental illness (SMI) and behavioral health challenges, helping them achieve their rehabilitation goals.
Key Responsibilities :
- Provide individualized rehabilitation support to clients, including developing and implementing rehabilitation plans.
- Conduct assessments to identify clients' rehabilitation needs and goals.
- Develop personalized rehabilitation strategies to help clients achieve their goals.
- Facilitate rehabilitation workshops and group sessions to promote skill development and recovery.
- Collaborate with a multidisciplinary team to ensure coordinated and effective care.
- Respond to crisis situations and provide immediate intervention as needed.
- Maintain accurate and up-to-date client records.
- Participate in rotational on-call duties to provide 24 / 7 support to clients.
