Role Overview:
- Primary Focus: Support member care delivery and skilled nursing facility (SNF) partners to provide high-quality, patient-centered care for I-SNP members.
- Main Responsibilities: Serve as the main contact for assigned facilities, coordinate activities with Network, Clinical, and Project Management teams, and communicate enrollment, clinical, and quality targets.
Sales Responsibilities:
- Function as a Sales Executive, responsible for achieving monthly enrollment goals through lead generation and sales presentations in compliance with CMS regulations.
Key Tasks and Responsibilities:
- Coordinate plan functions and lead efforts to deliver high-quality, cost-effective care.
- Maintain regular visits and communication with SNFs.
- Manage SNF contractual relationships and resolve operational issues.
- Conduct monthly data reviews and present best practices.
- Develop and enact facility-specific strategies and enrollment goals.
- Ensure facilities are prepared to launch and maintain I-SNP programs.
- Document market efforts and partner interactions.
- Participate in various meetings and ensure contract compliance.
- Educate SNF personnel on plan benefits and generate leads.
- Conduct benefit presentations and manage application processing.
- Support compliance programs and enforce company policies.
