Licensed Practical Nurse
Location: Montgomery County, NY
FLSA: Non-Exempt
Reports To: Residential Manager
Physical Requirements: Walking, climbing, reaching, bending, etc.
Hours of Work: Full time approximately 37.5 hours per week. Evening and weekend hours required.
Pay Range: The pay range is $20.81to $29.03
Job Summary: Providing on-site care to a medically frail person, attending doctor appointments, and monitoring and reporting healthcare status to RN.
Job Qualifications:
- Shall be currently licensed and registered as an LPN by the NYS Education Department
- Experience in clinical nursing setting and/or experience with the developmentally disabled preferred
- Valid NYS driver's license with no drug or alcohol related infractions or excessive points
- Employee must have reliable transportation available
Major Responsibilities:
- Provide Individual Support
- Provides all necessary self-care, and assist in directing nursing care
- Maintain Communication and Documentation
- Reports any concerns or medical problems to RN, as needed
- Completes medical documentation in a timely, accurate and professional manner
- Assist with medical paperwork, as needed (i.e.: ER forms, drug details, etc.)
- Assists with training of staff (under RN guidance)
- Administration of medications/treatments as ordered by the physician
- Assist with pertinent medical appointments, as needed
- Attend medical appointment, report information to physician and follow up after the appointment to RN for directions
- Monitors medical supplies and medications, orders as needed
- Liaison to the Residential RN that fall within the LPN scope of Nursing
- Monitor medical status of individuals and report to RN as required/directed

- Support DSP's in areas of concern/issues including medical emergencies, applying first aid, etc
- Ensure that all prescriptions are current
- Provide medical services to individual in the residential programs, in compliance with agency, local, state and federal regulations
- Attend mandated training as required, ensuring LPN license is current
- As assigned may include other tasks associated with the support of people in the residence
