Job Summary:
Liberty Community Connections, a leading organization in Long Term Services and Supports (LTSS), is seeking a dedicated and compassionate Service Coordinator to join our team. As a Service Coordinator, you will play a crucial role in assisting participants in obtaining essential services required by Community Health Choices (CHC). Our organization is contracted with all three Managed Care Companies approved by the Pennsylvania Department of Human Services.
Essential Responsibilities:
- Lead the participant-centered planning and service delivery process, ensuring its effective implementation.
- Identify, coordinate, and assist participants in gaining access to needed LTSS services and other Covered Services.
- Provide information to participants and facilitate access to a range of services and supports, including medical, social, housing, educational, and other resources.
- Inform participants about LTSS requirements, the PCSP process, service alternatives, and delivery options.
- Advocate for participant rights and assist with the complaint, grievance, and DHS Fair Hearing processes.
- Conduct reevaluation of the level of care annually or as needed in accordance with CHC requirements.
- Complete required visits, including pop-up visits and triggering events that may involve traveling to multiple consumers' homes in multiple counties.
- Work with participants to complete activities necessary to maintain LTSS eligibility.
- Advocate for continuity of services, system flexibility, integration, and proper utilization of facilities and resources.
Minimum Education and Experience Qualifications:
- Bachelor’s degree in social work, psychology, or related fields.
- At least three years of experience in a social service or health care related setting.
- Valid driver’s license required.
- Experience working with people with disabilities or seniors in need of LTSS.
- Experience working with a Managed Care company is highly desirable.
Job Type: Full-time
Pay: $40,000.00 - $45,000.00 per year
