Title: Supportive Housing and Community Supports

Hours/Shift: Monday-Friday Exempt

The Program Coordinator works with the Program Director to provide interventions to individuals who experience homelessness, housing crises, unemployment or under-employment. The Coordinator works independently to oversee and guide the provision of intensive community services and navigation to clients. Supervises full and part time staff that may work nontraditional hours. The Coordinator is responsible for compliance with funders for housing programs – HUD PSH, Shelter Plus Care, DOH RAP, Housing Choice Voucher and Section 8. The Coordinator is the service liaison to educational institutions with regard to service-oriented research projects. The Coordinator assumes responsibilities for other projects as assigned.

Essential Functions:

  • Supports the agency’s mission, philosophies, goals, planning objectives, special initiatives and supports their ongoing development and implementation.
  • Uses relationships, online sources, directories and other means to identify landlords and/or Property Managers with decent, safe, affordable housing in greater New Haven.
  • Incorporates evidence-based principles and practices in the provision of case management services including person-centered planning, integrated services, motivational interviewing, critical time intervention, trauma informed care, housing first and harm reduction.
  • Respect diverse cultural backgrounds and the impact on service provision.
  • Establishes policies and procedures for program in accordance with HUD and other applicable regulations.
  • Reviews and authorizes rental calculations and payments for Liberty and applicable partners, i.e., Shelter Plus Care.
  • Leads by example to have staff and programs reflect compliance with our values of customer service and individualized care with dignity, respect, hope and humanity.
  • Assists in organization efforts to conduct targeted outreach, engagement and eligibility verification for housing programs.
  • Solicits consumer input to formulate program direction, policies and procedures.
  • Maintains records in Case-worthy. Audits case records and HMIS entries.
  • Prepares and reviews compliance reports for funding sources, i.e., Annual Performance Reports (APRs) for HUD. Collects outcomes data and prepares quarterly reports on program performance.
  • Completes all internal and external designated reports/databases and maintain effective, accurate and current documentation and record keeping.
  • Maintains a current working knowledge of referral sources such as housing, medical, Medical/MH/SUD/Dental services, legal, HIV/AIDS client services, self-help groups and others to provide comprehensive resources to the program clients.
  • Ensures provision of referrals and supports during times of changing mental health, health, SUD and other client/tenants needs. Provides guidance to case management and employment staff to address changing needs.
  • Ensure creative methods are utilized to support client/tenant engagement.
  • Makes sure that the staff develop 1:1 case management relationships with clients/tenants to facilitate:
  • Housing stability
  • Income stability
  • Health stability
  • Supportive social networks
  • Appropriate orientation to Liberty Community Services program(s).
  • Creation of a client centered and empowering environment.
  • Development of effective resources/support teams with client/tenant.
  • Routinely audits case records and other documentation to make sure staff maintains required documentation. Coordinates peer reviews of case record on a regular basis.
  • Holds regular staff meetings to maintain timely communication and training.
  • Reviews and adheres to Liberty administrative procedures and employee handbook.
  • Represents Liberty and assigned programs to the community, agencies, and individuals.
  • Attends regular supervision with the Program Director.

Non-Essential Functions:

  • Transport clients – as necessary to housing, legal, medical and other appointments.
  • Participate in agency meetings/activities, assigned workgroups activities and other meetings as required.
  • Collaborate with IT on computerized data collection.
  • Other requests that may from time to time be made by the Program Director or Leadership Team to assist the agency's overall management.

Qualifications:

  • Bachelor’s degree in human services plus 5 years of experience non-profits, governmental or private for-profit agencies with community and housing services to vulnerable populations. Experience serving people facing homelessness, addictions, mental illness and poverty or experience is highly required. Excellent knowledge of services in Greater New Haven. Valid Connecticut Drivers license.
  • Creativity, flexibility and ability to work in a changeable and fast-paced environment.
  • Working knowledge of housing and human services in Greater New Haven. Experience with housing and homelessness services strongly preferred.
  • Strong organizational, written, computer, and communication knowledge and skills a must.
  • Valid CT driver’s license required.
  • Bilingual capabilities a plus.
  • Three strong work-related references (upon request).
