Job Summary
The Resident Experience Coordinator (REC) is a compassionate and organized Licensed Practical Nurse responsible for enhancing the overall quality of life for residents by ensuring a positive, person-centered experience. This role involves close interaction with residents, families, and care teams to monitor and improve resident satisfaction, coordinate personalized care plans, and promote an environment that aligns with the mission of resident-centered care.

Qualifications:
- LPN license required
- skilled nursing experience preferred
Key Responsibilities:
- Resident-Centered Care Coordination
- Serve as an additional point of contact for residents and families, actively listening to and addressing their needs, concerns, and preferences.
- Develop and implement individualized resident experience plans, communicating these clearly across the care team.
- Conduct regular check-ins with residents to monitor satisfaction and assist with resident care plans based on interactions and resident or family feedback.
Care and Wellness Management:
- Assist in daily clinical tasks as needed, including administering medications, observing resident conditions, and performing basic nursing duties to assist in the resident experience.
- Collaborate with RNs, CNAs, and other healthcare providers to ensure cohesive care that supports residents’ physical, emotional, and social well-being.
Serve as point of contact post discharge at specified timeframes to ensure post discharge services are in place and resident is succeeding in the community.
Family Communication and Support:
- Serve as a liaison for families, updating them on resident progress, addressing questions, and promoting family involvement in resident care.
- Coordinate family meetings, both routine and as needed, to discuss resident care and satisfaction.
Quality Assurance and Documentation:
- Maintain accurate, up-to-date records of resident preferences, satisfaction surveys, care plans, and daily interactions.
- Participate in quality assurance programs, helping identify areas for improvement in resident care and satisfaction.
Team Collaboration and Training:
- Work closely with staff across departments to implement resident-centered initiatives and improve service quality.
- Provide training and guidance to staff on best practices in resident engagement and communication.
Actively contribute to other resident care initiatives and responsibilities, always with a focus on providing the highest level of resident care.
