**Key Responsibilities:**
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide direct patient care and coordinate with other healthcare professionals to ensure seamless care delivery.
- Collaborate with the interdisciplinary team to develop and implement treatment plans.
- Maintain accurate and detailed records of patient care and progress.
**Requirements:**
- Current RN licensure in state of practice.
- 2 years of clinical nursing experience, preferably in hospice care.
- Strong knowledge of nursing principles and practices, including reimbursement guidelines.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
