We are seeking an experienced HOSPICE REGISTERED NURSE (RN) to join our team on a full-time basis in Chatham & Lee Counties.
Job Description
- Apply your knowledge of nursing theories and concepts to deliver exceptional patient care.
- Coordinate physical care of the patient with family, patient, physician, and Hospice team members.
- Communicate effectively with patients/families at all levels and with agency personnel.
- Attend and participate in IDT meetings and quality reviews as scheduled.
Requirements
- Current RN licensure in the state of practice, successful completion of a nursing education from an approved school of nursing, and two years of clinical nursing experience.
- Knowledge of the framework, function, and organization of home health and hospice care programs and the nurse's role within the patient care team.
- Working knowledge of principles and practices of nursing according to nursing practice act, agency policies and procedures, and reimbursement guidelines.
- 12 hours of continuing education annually and CPR certification.
