Job Description:
- We are seeking a registered nurse to join our team as a patient care coordinator. In this role, you will be responsible for coordinating the delivery of care to our residents, working closely with our healthcare team to develop and implement individualized care plans.
- You will also be responsible for supervising nursing assistants and ensuring that they are meeting the needs of our residents.
Responsibilities:

Requirements:
