HOSPICE CLINICAL MANAGER – TRAVEL

(Covering North Carolina Statewide)
JOB SUMMARY:
- Provide coordination of referrals and coordination of patient care.
- Provides oversight of all patient care services and personnel.
- Assure that patient needs are continually assessed.
- Assure the development, implementation and updates of the individualized care plans.
- Provide appropriate documentation for both patient care and staff performance.
- Assist Administrator as necessary.
- Provide direct patient care as necessary to include case management.
- When the Administrator is not available, the Clinical Manager will be designated to assume the responsibilities of the Administrator. The Administrator and the governing body will authorize this in writing.
- Accepts assignments as an interim Clinical Manager when the need is identified throughout all of Liberty Home care's locations.
JOB REQUIREMENTS:
- Requires current RN licensure within state of Clinical Office location, and successful completion of nursing education from an approved school of nursing.
- Requires five (5) years of clinical nursing experience. Prefer (3) years of hospice experience.
- Requires in-depth knowledge of hospice regulations.
- Must be able to independently organize work, provide appropriate case management, cost effective home care and/or hospice services, and timely, accurate documentation of said care.
- Knowledge of Infection control, safety and injury prevention strategies and actions
- Knowledge of quality improvement principles and strategies.
- Clinical knowledge about a wide range of diagnoses and conditions, medications.
- Must possess strong critical thinking and communication (oral and written) skills and be literate in the English language.
- Must be able to use a personal computer and have a working knowledge of a variety of computer applications (Word, Excel, Window, etc).
- Must complete a minimum of twelve hours of continuing education annually.
- Must be CPR certified.
- Must have a valid driver's license.
- When traveling, overnight stays are required.
