We are currently seeking a qualified candidate to fill this critical role. As a Home Health LPN, you will play a vital part in ensuring that our patients receive the highest standard of care possible.
Your Key Responsibilities Will Include:
- Providing direct patient care, encompassing a range of tasks and duties designed to promote patient recovery and overall well-being
- Collaborating with other healthcare professionals, including registered nurses and patient care coordinators, to deliver coordinated care
- Following established care plans to ensure that patients receive consistent and high-quality care
- Documenting patient care activities and outcomes in a clear and concise manner
- Responding to changing patient needs and adapting care plans accordingly
