About the Role
We are seeking an experienced MDS Coordinator with a valid RN license to join our team on a full-time basis. The ideal candidate will be responsible for:
- Maintaining and following schedules for all MDS assessments.
- Coordinating the completion of MDS assessments by various disciplines.
- Reviewing MDS data for accuracy and collaborating with staff as needed to ensure precision.
- Completing MDS assessments and inputting data into the computer system.
- Verifying timely transmission and approval of assessments by the State.
- Correcting rejected records and preparing them for re-submission.
- Completing CAAs according to state guidelines.
- Developing Resident Care Plans (RCP) in collaboration with interdisciplinary care teams.
- Scheduling and conducting regular RCP meetings.
- Communicating RCPs to relevant staff members and addressing non-compliance issues.
- Fostering understanding and compliance with RCPs across departments.
Requirements
- Registered Nurse degree from an accredited School of Nursing.
- Current, valid RN license from the North Carolina Board of Nursing.
- Ability to read, understand, and adhere to facility policies, procedures, and local/state/federal requirements.
- Experience with MDS-RAP and Care Planning functions.
- Prior experience with RUG-IV and/or MDS 3.0 is preferred.
