Overview:
Liberty Medical Care Services is seeking a highly skilled Nurse Health Professional to join our team.
Responsibilities:
- Directs and coordinates nursing care to ensure quality service delivery.
- Conducts physical assessments and annual evaluations for assigned patients.
- Completes acute visits to assess and diagnose patient conditions.
- Documents patient assessment and plan of care in the electronic medical record.
- Collaborates with physicians and staff to ensure seamless care coordination.
Requirements:
- Current, unrestricted valid Nurse Practitioner license from the Board of Nursing/PA License from the Medical Board in the state of assignment or the ability to obtain.
- DEA certificate current or the ability to obtain.
- Clinical background in adult, family or geriatric specialties preferred.
- Strong communication and relationship building skills preferred.
