Rehabilitation Director (PT/SLP/OT)
Full Time, Days
As part of our multidisciplinary team, you will play a pivotal role in delivering exceptional rehabilitation experiences for patients and their families. You will be responsible for developing and implementing individualized treatment plans, working collaboratively with colleagues to ensure seamless care transitions, and leveraging evidence-based practices to drive optimal patient outcomes.
Key Responsibilities:
1. Develop and implement comprehensive rehabilitation programs tailored to the unique needs of each patient.
2. Collaborate with interdisciplinary teams to ensure holistic care and effective communication.

Education Requirements:
Graduate of an accredited therapy program.
Licensure/Certification:
Valid licensure in the state where the SNF is located.
Experience:
Previous experience in rehab SNF settings and management roles preferred.
