This role involves providing Speech Language Pathology services to patients in their homes. The successful candidate will be responsible for conducting evaluations and assessments, developing treatment plans, and implementing interventions to improve patients' speech, language, and cognitive skills.
Key Responsibilities
- Conduct comprehensive evaluations of patients' speech, language, swallow, and cognitive abilities.
- Develop individualized treatment plans that address patients' specific needs and goals.
- Implement interventions and therapies to improve patients' speech, language, and cognitive skills.
- Document patients' progress and outcomes regularly.
- Communicate effectively with patients, families, and interdisciplinary teams to ensure seamless care coordination.
Qualifications
- Bachelor's or Master's degree in Speech Language Pathology.
- Certificate of Clinical Competence from the American Speech-Language-Hearing Association.
- Current licensure from the state in which employed.
- Ability to work independently and as part of a team.
