Due to organizational growth, Liberty Healthcare is hiring for several new full-time Options Counselor positions throughout Kansas.
As an Options Counselor, you will actively contribute to a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities.
Service objectives include:
- Functioning as the “go to” resource for individuals who are seeking information on long term care and other community-based services and supports
- Providing person-centered planning to individuals in a way that supports their informed decision making and ability to secure services which will enable them to live as independently as possible
Your day-to-day responsibilities as an Options Counselor will involve:
- Fielding a high volume of phone calls and email requests
- Talking with individuals and their families about their needs
- Providing information and answering questions
- Connecting people with appropriate services and supports
- Traveling throughout the region to meet with individuals in-person
- Documenting interactions, services and relevant information in computer systems
This is a hybrid position which will require you to travel to various locations throughout the region and allow you to partially work from your home office.
As a full-time employee of Liberty Healthcare in this role, you can expect:
- Annual compensation: $50,000
