What are the responsibilities and job description for the RN Field Supervisor position at Liberty Home Care?
Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team on a Per Diem basis! Per Diem shifts available Monday - Friday 9am-5pm.
Duties & Responsibilities
Provide assessment of newly referred patients and assess health needs in home care setting.
Work with patient to create a patient centered plan of care.
Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician / family.
Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.
Provide supervision and training of PCA / HHA staff.
Qualifications
NYS Licensed Registered Professional Nurse.
Minimum of Associate's Degree required.
One year Nursing experience required.
Home care experience a plus.
