What are the responsibilities and job description for the Care Manager/RN position at Liberty Hospital?
SUMMARY: Assumes primary responsibility for coordination of patient care services by all relevant disciplines. Serves as a patient advocate in working with the team by facilitating clear communication, explaining role to patients and appropriate family members and others. Anticipates day of discharge and disposition. Creates a plan of care that outlines the key interventions and outcomes to be achieved each day of the patient stay. Coordinate the key interventions among and between members of the interdisciplinary care team. Partners with attending and consulting physicians to ensure smooth progression of care from admission to discharge. Utilizes clinical and assessment skills to provide cost effective patient care management. Coordinates services for the patient to ensure timely movement through the health care system
Required Education/Training: Graduate from a nursing program
Required Experience: Minimum five (5) years recent hospital-based acute care experience
Required Licensure/Certification/Registration: Current Registered Nurse license with eligibility to work in the state of Missouri
Preferred Education/Training: BSN; graduate from an accredited nursing program
Preferred Experience: Case Management experience; excellent verbal and written communication skills
Preferred Licensure/Certification/Registration: Advanced practice certification; CCM, CPUR, CPUM or equivalent Case Management certification
