Liberty Village of Peoria is seeking a qualified MDS Care Plan Coordinator for our beautiful retirement community. MDS Coordinator Duties:
- Complete care plan and resident care information sheet with new resident admissions
- Complete assigned assessments, sections of the MDS, and CAA summaries with new resident admissions, quarterly, upon significant changes, and annually
- Complete discharge and re-entry assessments
- Review and update care plan and resident care plan information sheet quarterly and as needed
- Keep and monitor RUGs worksheet for all residents on Medicare
- Schedule and complete all Medicare MDS
- Participate in Medicare meeting
- Monitor and complete all corrections on MDS

MDS Coordinator Requirements:
- Current Illinois LPN/RN License
- 2-4 Years of MDS Experience in a Long-Term Care Setting
- Ability to Pass a Criminal Background Check
