Position Overview:
We are seeking a highly skilled and compassionate Advanced Practice Nurse (APN) to join our team at our drug and alcohol/mental health facility. The APN will play a crucial role in providing comprehensive medical care to our patients, collaborating with our interdisciplinary team to develop and implement treatment plans, and supporting patients on their journey to recovery.
Responsibilities:
- Conduct comprehensive assessments of patients, including physical exams, psychiatric evaluations, and substance abuse history.
- Develop and implement individualized treatment plans in collaboration with the interdisciplinary team, including medication management and therapeutic interventions.
- Provide education and support to patients and their families regarding treatment options, medication management, and strategies for maintaining sobriety and managing mental health symptoms.
- Monitor and evaluate patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
- Collaborate with other healthcare professionals, including psychiatrists, psychologists, social workers, and addiction counselors, to coordinate care and provide integrated treatment services.
- Participate in multidisciplinary team meetings, case conferences, and clinical supervision sessions to review cases, discuss treatment strategies, and ensure continuity of care.
- Maintain accurate and up-to-date medical records in accordance with regulatory standards and organizational policies.
- Participate in quality improvement initiatives and program development activities to enhance the delivery of services and improve patient outcomes.
Qualifications:
- Master's or Doctoral degree in Nursing from an accredited program.
- Current license to practice as an Advanced Practice Nurse (APN).
- Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) or Family Nurse Practitioner (FNP) with a specialization in psychiatric/mental health.
- Minimum of 1 years of experience working in a psychiatric/mental health and/or addiction treatment setting.
- Strong clinical assessment and diagnostic skills, with the ability to formulate and implement comprehensive treatment plans.
- Excellent communication and interpersonal skills, with the ability to establish rapport with patients and collaborate effectively with colleagues.
- Demonstrated knowledge of evidence-based practices in addiction treatment and mental health care.
- Ability to work independently and as part of a multidisciplinary team in a fast-paced and dynamic environment.
