- Assesses the bereaved prior to and after the death of the patient.
- Manages the bereavement services program utilizing Hospice staff and volunteers.
- Plans, implements and facilitates bereavement support groups for hospice families and the community.
- Provides monthly reporting of bereavement services to Psychosocial Coordinator, Operations Manager and Patient Care Coordinator.
- Assists in providing community education on end of life issues as appropriate.
- Educates staff on ongoing grief resources, education and issues for increased knowledge.Job Requirements :
- Bachelor's degree in human services field such as social work, psychology, counseling or related field with experience and education in death and dying and the grief process; Masters degree preferred.
- Proficiency in writing and oral communication.
- Knowledge and skill with computer software programs to include Word, Excel.
- Good organizational skill and the ability to work efficiently with people.
- Experience in group facilitation and support intervention.
