Occupational Therapist - 0.2 FTE PT - LHS
Job Description: Seeking a State of Illinois licensed Occupational Therapist to collaborate as an engaged member of IEP and 504 Accommodation Plan teams to provide school-based OT related services to eligible students based on educational need with a focus on access to and success in the educational environment.
Assignment: 0.2 FTE at Libertyville High School
This assignment is a 1-day per week assignment, and the district is open to flexible scheduling (i.e., two ½ days/week, 1 full day/week, or other potential iterations with department chair approval)
Reports to: Director of Special Education and Department Co-Supervisors
Qualifications:

- Graduate of an accredited occupational therapy program with entry-level credentials
- Current licensure to practice occupational therapy issued by the Illinois Department of Financial and Professional Regulation
- Experience providing OT related services in high school setting and transition setting preferred
- Valid Illinois driver’s license and access to a reliable, insured vehicle in order to drive between schools and service provision locations in the community

- Implement the professional practices and standards associated with the discipline of OT (e.g. IDFPR, ISBE, CEC)
- Implement District 128’s policies and procedures for special education referrals and service delivery
- Support planning and service delivery options that allow students with OT needs to have equal access to curriculum, school activities and facilities
- Conduct screening and diagnostic assessments and provide services as needed for students referred through the District’s systematic child find process
- Provide interpretation of assessment results and recommendations for educational team consideration, including from private providers or non-District entities, as appropriate
- Serve as a member of multi-disciplinary teams in student assessment, placement and service delivery determination, progress review, and service exit determination
- Establish and maintain effective interpersonal relationships with students, parents, and professional colleagues and contribute in a positive and constructive manner during interactions with teams and in meetings
- Provide training to necessary members of a student’s educational team, including family members, to implement or reinforce and monitor student use of OT strategies, interventions or exercises for generalization and growth
- Complete special education components of OT related service eligibility including functional updates, present levels of progress for annual IEP goals, and make school- and age-appropriate related service delivery recommendations
- Maintain related service logs and data to complete written goal progress updates as required throughout the school year and report on each student’s progress toward selected goals annually in writing
- Attend Team Meetings and IEP meetings for students on related service caseload as scheduled
- Provide direct and consultative, evidence-based therapeutic services to identified students with occupational therapy needs in 1:1, small group, classroom- and/or community-based settings as appropriate to successful growth in the student’s functional needs
- Collaborate with school teams to provide interdisciplinary therapeutic interventions for students with sensory and social-pragmatic challenges that foster increased independent self-awareness of strategies and accommodations to engage in society safely and with self-regulation
- Evaluate and identify the need for sensory diet plans and school- or community-appropriate sensory tools and strategies that proactively optimize the student’s ability to engage in their educational activities
- Identify the need for adaptive equipment and assistive technology tools and safety plans for functional needs for optimal educational participation.
- Utilize technology in the provision of OT related services, including Tier I 1:1 device as well as assistive and/or augmentative communication device, when appropriate
- Perform other duties as assigned
