Summary
As an MDS Coordinator at Life Care Centers of Cape Girardeau, you will play a vital role in ensuring the highest standards of patient care through effective management of the Minimum Data Set (MDS) process. This position is crucial for maintaining compliance and enhancing the quality of care provided to our residents.
Responsibilities
- Coordinate and manage the completion of MDS assessments for residents in accordance with federal and state regulations.
- Ensure accurate clinical documentation to support reimbursement and quality measures.
- Collaborate with interdisciplinary teams to develop comprehensive care plans based on assessment data.
- Monitor resident progress and adjust care plans as necessary to meet changing needs.
- Provide training and support to staff on MDS processes and documentation standards.
- Participate in quality improvement initiatives to enhance patient outcomes.
Requirements
- Registered Nurse (RN) license required.
- Must have LTC experience.
- Experience in case management or MDS coordination preferred.
- Strong knowledge of clinical documentation improvement practices.
- Excellent communication skills for effective collaboration with healthcare teams.
- Ability to manage multiple tasks efficiently in a fast-paced environment.
